Kensei Glitch - Ability Just Stops Working

Is anybody else having the issue were their Kensei slash will just stop doing any damage?

I want to say it happens at least 1 out of 5 times trying to use the ability. I will charge the ability and for the next 3 slashes no damage until I recharge the ability again.

Its like the servers cant keep up with the character/unity client.

Kensei Build for reference:

Sheath: Volcanic Sheath
Armor: Resurrected Warrior’s Armor
Ring: Bloodshed Ring

It may be a quirk with volcanic sheath.
I would certainly file a bug report to Deca.
