Not sure how common this is, but I just “finished” a woodlab and the boss never went into it’s second phase. You can also see that everyone had no clue what just happened, as well as myself.

This happens when you overdamage it and kill it before it has a chance to form. Since he never reaches his final form you can’t get loot from it.
