I thought it was something like thirty seconds after someone got within a number of tiles. Probably wrong, though.

@Gammagamer is right. Proximity sets off their countdown timer to disappearance. If nobody goes near one it will remain indefinitely (until the realm closes).

That, but I sometimes also remember seeing a healthbar below a Realm-spawned portal, as though there was some extra entity there…
It might just be a slight oversight, though.

Due to the rather large size of wormhole sprites, it’s possible that a monster was right behind it so that its body is hidden but its HP bar is not.

Aye, but I stood around and didn’t notice anything. It was a half-full HP bar, no enemy in sight, and a “standard” hitsound when hitting it (the sort that Trap Elements have)
