This wakizashi does not deal any damage, and doesn’t affect enemies whatsoever, as it is a vanity item. It inflicts the exposed status effect on the player for 10 seconds, and replaces the player skin with the following sprite:

Trivia
The name of the item references an acronym commonly used on the internet. NSFW, which stands for Not Safe For Work is often used to tag sensitive content which may contain violence and/or sexual imagery that should not be viewed in a workplace environment. In the context of the item, the player character is quite literally getting exposed (or more specifically, getting their body exposed) upon use. The eggplant particle that is spawned when using the ability is a reference to the eggplant emoji (🍆) that is often used on social media as a less graphic alternative to male genitalia.
